三、 投標人資格要求Bidderqualificationrequirement

3.1本次招標不接受聯(lián)合體投標。

3.2投標人要求具有獨立法人資格，為此采購項目的專業(yè)生產、制造、安裝、調試的制造商或代理商或貿易商，對其提供的貨物和服務負全部責任；在法律和財務上獨立，合法運作并獨立于招標人（招標機構）的制造商或代理商或貿易商。如投標人為代理商，需提供品牌有效授權書。

3.4投標人需在投標文件中如實寫明中標后該批貨物的生產地址，如生產、制造加工基地，如與投標人營業(yè)執(zhí)照中的注冊地址不同，需提供生產、制造加工地的資質，不接受生產分包。

3.5財務要求：

投標人應財務狀況良好（其中資產負債率小于**0%），沒有處于財產被接管、破產或其他關、停、并、轉狀態(tài)；投標人需承諾不存在進入清算程序、被宣告破產或其他喪失履約能力的情形（投標人需在其投標文件中提供無上述情形的承諾，見“第四章格式VI-**．制造商承諾書格式第5條”）；投標人須提供****年度的經會計師事務所或審計機構審計的財務報告（須由注冊會計師簽字蓋章）。財務報告內容至少應包括：包括資產負債表、現(xiàn)金流量表、利潤表。

3.6業(yè)績要求：投標人需提供****年至****年以來類似產品有效業(yè)績（不低于1個有效業(yè)績），業(yè)績表中產品的性能或技術參數(shù)應與本次采購所提供產品相近。業(yè)績表至少應包括工程名稱、產品規(guī)格型號及主要技術參數(shù)、供貨數(shù)量、簽訂合同時間(供貨年份)、業(yè)主名稱、業(yè)主聯(lián)系人及聯(lián)系方式。提供的合同復印件至少應包括：封頁；簽字蓋章頁；合同中包含買方名稱、物資名稱、規(guī)格型號及技術參數(shù)、供貨數(shù)量、簽訂時間的關鍵頁；合同發(fā)票復印件至少應包括與合同對應的1張發(fā)票”。
