投標人資質要求:

【★資格條件】一、資格要求：1、營業(yè)執(zhí)照：投標人具有合法有效的企業(yè)法人營業(yè)執(zhí)照、稅務登記證及組織機構代碼證或證照合一的營業(yè)執(zhí)照，投標時需提供原件掃描件（原件備查）。投標人為事業(yè)單位的，應具有合法有效的事業(yè)單位法人證書，投標時需提供原件掃描件（原件備查）。投標人為分公司的，提供具有合法有效的營業(yè)執(zhí)照和上級法人單位授權書（授權該分公司投標），認可該分公司和上級法人單位的資質、資格和業(yè)績，不認可同一上級法人單位的其它分公司的資質、資格和業(yè)績，投標時需提供原件掃描件（原件備查）。分公司與上級法人單位只可一家參與投標，同時參與投標的，投標均無效。 如果投標人為中國境外注冊公司，需提供有效的公司登記注冊證明。2、制造商要求：投標人應為本次投標所投產品的制造商。本次招標不接受代理商或貿易商投標。1）制造商無直銷模式，且投標人為制造商同一集團負責銷售投標所投產品的全資子公司、控股子公司（投標人應提供制造商或其上級單位證明上述事項的證明文件），則視投標人為制造商。不能再將此貨物同時授權給其他投標人參與投標，否則相關的所有投標將被拒絕。2）本次接受制造商的母公司或全資的銷售公司、制造商控股的銷售公司或者隸屬于同一集團的銷售公司或全資公司，參與投標的，視為制造商身份投標，投標時需提供相應的證明文件。同一制造商僅允許一家投標人參與投標，否則相關的所有投標將被否決。3）投標人所投產品為國外品牌或國外品牌在國內生產的，如果投標人所提供的貨物不是投標人自己制造的，投標人應得到同一集團貨物制造商同意其在中國為本次投標提供該貨物的合法正式授權，或者母公司的證明或認可。滿足上述1）或者2）或者3）要求的投標人視為制造商。投標人（制造商）不能再將此貨物同時授權給其他投標人參與投標，否則相關的所有投標將被拒絕。投標時提供符合要求的證明投標人為制造商證明相關材料，材料要求詳見格式IV-**制造商證明相關材料。3、關聯(lián)關系要求：如出現(xiàn)不同投標人的負責人為同一人或存在控股、管理關系的情況，相關投標均無效，招標人隨時有權做出拒絕投標、取消投標資格、取消授標、不簽訂合同或在合同簽訂后終止合同等決定，相關投標人須無條件接受，且由此做出的任何損失由對應投標人承擔。4、體系認證要求：投標人所投產品的制造商投標時需提供合格有效的GB/T****1（ISO****）質量體系認證證書，并可在中國國家認證認可監(jiān)督管理委員會網站(http://www.cnca.gov.cn/)核實。如果有國家相關部門發(fā)布的最新體系標準，以最新體系標準為準。投標時需提供原件掃描件（原件備查）。注：如所投貨物的制造商提供的質量體系認證證書是非中國的第三方認證機構頒發(fā)的，則網站查詢不適用。Qualification Requirements(Performance track record will be released to all bidders during bid opening):1.Business license:The bidder has a legal and valid business license of enterprise legal person, tax registration certificate and organization code certificate or a business license with one certificate, and shall provide a scanned copy of the original (original for future reference) when bidding.If the bidder is a public institution, it shall have a legal and valid certificate of legal person of the public institution, and shall provide a scanned copy of the original (original for future reference) when bidding.If the bidder is a branch company, it shall provide a legal and valid business license and a letter of authorization from the superior legal entity (authorizing the branch company to bid), recognize the qualifications, qualifications and performance of the branch company and the superior legal entity, and not recognize the qualifications, qualifications and performance of other branches of the same superior legal entity.
