Brief specification description or basic overview of the project:Package 6: Outsourced Testing Service - Department of Gynecology， Service Location: Hospital campuses designated by the Purchaser （No. **0 Jimo Road, Pudong New District, Shanghai, and No. **** Yuntai Road, Pudong New District, Shanghai ）.
The relevant provisions of the tendering documents shall govern the specific project content, procurement scope, and specific requirements that must be satisfied. Partial bidding for any individual item within the awarded package is not permitted. A comprehensive quotation covering all items in the package shall be submitted.
合同履約期限：服務(wù)期限一年，預(yù)計自****年3月**日至****年3月9日，具體以合同簽訂日期為準。

(b)落實政府采購政策需滿足的資格要求：本次招標(biāo)若符合政府強制采購節(jié)能產(chǎn)品、鼓勵環(huán)保產(chǎn)品、扶持福利企業(yè)、促進殘疾人就業(yè)、支持中小微企業(yè)、支持監(jiān)獄和戒毒企業(yè)等政策，將落實相關(guān)政策。本項目面向大、中、小、微型等各類供應(yīng)商采購，對小型和微型企業(yè)投標(biāo)人貨物或服務(wù)的價格給予**%的扣除，用扣除后的價格參與評審。其要求標(biāo)準詳見《政府采購促進中小企業(yè)發(fā)展管理辦法》（財庫[****]**號）、《關(guān)于進一步加大政府采購支持中小企業(yè)力度的通知》財庫〔****〕**號文中相關(guān)規(guī)定。
(b)Qualification requirements to be met to implement government procurement policies: If this bidding complies with the government's mandatory procurement of energy-saving products, encouragement of environmentally friendly products, support for welfare enterprises, promotion of employment for the disabled, support for small and medium-sized enterprises, and support for prisons and drug rehabilitation enterprises, relevant policies will be implemented.
This project is open for procurement to suppliers of all types, including large, medium, small and micro-sized enterprises. A **% price deduction will be applied to the bid prices of goods or services submitted by small and micro-sized enterprise bidders, and the prices after deduction will be used for evaluation.
The requirements and standards are detailed in the "Administrative Measures for Government Procurement to Promote the Development of Small and Medium-sized Enterprises" ( [****] No. **) and the "Notice on Further Increasing Government Procurement Support for Small and Medium-sized Enterprises（[****] No. **）.
(c)本項目的特定資格要求：3、法人的分支機構(gòu)以自己的名義參與投標(biāo)時，應(yīng)當(dāng)提供由法人出具參加本項目的授權(quán)書；
4、單位負責(zé)人為同一人或者存在直接控股、管理關(guān)系的不同供應(yīng)商，不得參加同一合同項下的政府采購活動；為本項目提供整體設(shè)計、規(guī)范編制或者項目管理、監(jiān)理、檢測等服務(wù)的供應(yīng)商，不得參加本項目的采購活動；
5、投標(biāo)人具有有效期內(nèi)的《醫(yī)療機構(gòu)執(zhí)業(yè)許可證》；
6、本項目不接受聯(lián)合投標(biāo)；
(c)Specific qualification requirements for this program:(iii)When a branch of a legal person participates in the bidding in its own name, it shall provide a letter of authorization issued by the legal person to participate in the project;
(iv)Different suppliers whose heads are the same person or who have direct controlling or management relations shall not participate in government procurement activities under the same contract; suppliers who provide overall design, specification compilation, or project management, supervision, testing and other services for this project shall not participate in procurement activities for this project;
(v)Bidders shall hold a valid Medical Institution Practice License.
(vi)This project does not accept joint bidding;
